Edit 1/16: Name spelling correction. Zackary to Zachary.

HURRICANE, UT – A 24-year-old Provo man, Zachary Mansfield, is dead following an incident that happened on May 14, 2019, at Quail Creek State Park.

Zackary went shore fishing at about 7:30 p.m. with a friend but was alone at the time of the accident.
